You have lost your credit card. Write to the manager of your bank. Explain where and how you lost the card and any other relevant details. Ask the manager to cancel the old card and to send you a replacement.

Hello Sir / Madam,
I am your potential credit card customer for the past 2 years. I am writing to you in order to report loss of my credit card last night. I was in an office party, last night and was able to catch the last bus which goes on the way to my residence.

Since the bus was having heavy crowd I was not in a position to realize that someone has already stolen my wallet without my knowledge. As soon as I got down at my bus stop, I have contacted your 24 hours customer support to block my cards to avoid misuse by others.

To my knowledge, I have paid all my dues on time and don't have any balance dues at present.

Kindly consider this as my formal written complaint to the bank and take necessary action immediately to cancel the card and issue me a replacement one.

Please find the required details about my card for your reference.

Expecting your prompt action with this regards. Thanks.

Well this is not really necessary for your letter. Your letter is not intended for impressing him about your creditworthiness or anything. You need to lodge a complaint that you lost your credit card. Follow these steps when you attend to this task;

1. Identify the type of letter you are being asked to write.
2. Open and close the letter correctly (Formal - Dear Sir, Semi formal - Dear Mr Silva, Informal - Dear Tom) (Formal - Yours sincerely, Semi Formal - Your sincerely, Informal - Best Regards)

3. Open a formal and semi-formal letter with a formal sentence
4. Open an informal letter with a general, friendly paragraph
5. Identify the main purpose of the letter.

Wish you pay attention to what dumi has suggested. She highlights some of very important aspects in letter writing.

.... I think this is not really necessary to mention. Dedicate one paragraph to describe how you lost your credit card. There, you need to skim the details as to what are really necessary. The manager is not a police officer and would not be interested in every little detail about the incident. So, tell him how, when and where you lost the card. That's enough.
