She has done her O level from your school and left the school on 11th August 2011. Therefore the security fee check you issued is named after my husband, Mike Parker. Due to certain reasons I want you to kindly address the check to my name, Linda Parker. My NIC number is ##############. I'm also attaching a copy of my NIC for your convenience. Dear Madam , REF : CHANGING OF NAME ON THE CHEQUE. The security fee cheque which you issued under my husband's name, Mike Parker. Due to certain reasons, I would like to request you address the cheque to my name. I have attached a copy of NIC for your further reference.
